ADOPTION CONTRACT Adoption Coupon Book
I, the ADOPTER agree to: Free 8lbs Bag of Pro Plan
1. Provide proper and sufficient food, water, shelter, all necessary medical care* and kind treatment for the dog at all times.
2. Have the dog immunized (including rabies and distemper shots) and examined ANNUALLY, at the owner’s expense.
3. Keep the dog current on heartworm preventative and use approved flea/tick control as needed.
4. License the dog in the city or town in which the ADOPTER resides.
5. Inform MidAmerica Boston Terrier Rescue and HomeAgain of any address and/or telephone change in the event of a move.
6. Keep means of identification on the dog at all times.
7. Give adequate time for the dog to adjust to its new surrounding and his/her new owners.
8. Contact MidAmerica Boston Terrier Rescue immediately for assistance and/or counseling if any problem(s) should arise.
9. Not sell, abandon, or give the dog away to anyone else. If the owners can no longer keep the dog MABTR must contacted.
10. Not permit the dog to run at large or to become a public nuisance.
11. Not allow the dog to be used for the purpose of experimentation or fighting.
12. Immediately retrieve the dog from any animal shelter when notified the dog is being held at such a location.
13. Notify MidAmerica Boston Terrier Rescue immediately if the dog is lost, stolen, or dies.
14. Provide suitable confinement for the dog inside and outside of the home. Keep the dog on a leash when outside of a fenced area.
15. Do not leave the dog outside for any length of time while you are away from home. This breed is not an outdoor dog.
16. Arrange for suitable care if the owner will be on a prolonged absence from the home. MABTR does offer pet sitting services.
17. Assert no claim, charge or demand of any kind against MidAmerica Boston Terrier Rescue or its agents for any expenses that have
been incurred by the ADOPTER, including veterinary fees, in connection with the dog once the adoption is finalized.
18. Understands that MABTR cannot predict what medical or temperament problems a dog may have in its lifetime.
19. Accept sole responsibility for any illness or injury occurring after adoption.
20. Accept the dog as is and assume all risk of ownership of the dog, including risk of injury or damage caused by the dog, including bites.
21. Receive periodic follow-up telephone calls and inspections from the Rescue Agent to be assured of compliance to all terms of this agreement.
22. If the terms and conditions of this Adoption Agreement are not upheld, MidAmerica Boston Terrier Rescue reserves the right to terminate the
agreement and to reclaim the dog at any point of the dog’s life with no legal commitment to any monetary refund.
If for any reason, the adoption is not found to be satisfactory for the dog and/or the ADOPTER, the dog must be returned to MidAmerica Boston
Terrier Rescue. No refund will be made for returned dogs after the fourteen-day trial period.
The adoption fee is $________. All fees collected are used to cover rescue expenses, including foster care costs and veterinary bills incurred by
MidAmerica Boston Terrier Rescue. For legal purposes the value on this adopted dog is no less than $500. The rescued dog has been examined
by a veterinarian, received a heartworm test, started on heartworm preventative, and is up-to-date on shots including Rabies and Distemper. All
dogs are spayed and neutered before placement in the ADOPTER’s home. Any other medical care provided by MABTR is documented in the
rescued dog’s profile and vet records, which have all been given to the new owners.

I fully and completely release MidAmerica Boston Terrier Rescue and its agents/volunteers harmless from any claim,
cause or demand as a result of my adoption, ownership, care, maintenance or retention of the dog.
By signing below, I acknowledge that I have read and fully understand the terms and conditions of the adoption agreement and
accept this document as a contractual agreement, which I will comply with.
